SUNY Cortland Athletics

ATHLETIC TRAINING/MEDICAL FORMS

All student-athletes must complete additional athletic pre-participation medical requirements before they will be allowed to participate in any Cortland intercollegiate athletic program. These requirements are set by the NCAA and are separate from the requirements for admission to the college.

* If you are under the age of 18, a parent/guardian signature is required on several forms. Please complete the online forms as instructed, but contact Head Athletic Trainer Harry Campbell at harry.campbell@cortland.edu to receive hard copies of the forms for parent/guardians to sign.

Student Health Services

* Pre-Participation Physical Evaluation Form (PDF) 
(must be completed by your Primary Care Physician within six months from the start of the season. Please mail/fax to Student Health Services, SUNY Cortland, Van Hoesen Hall, Room B-26, Cortland, NY 13045, or fax 607-753-2486)   
* Immunization and Health History Forms (accessible on myRedDragon Student Health Services Web Portal)
* Sickle Cell Trait Lab Test results (Must be faxed to the SHS along with immunization and Health History Forms)

Athletic Training Forms (accessible through myRedDragon)

1. Go to SUNY Cortland "My Red Dragon" site:  myreddragon.cortland.edu
2. Navigate to the student tab located at the top of the page
3. At the bottom right side of the page, under Student Health Services, click on "Student Health Web Portal"
4. Click on "Forms" located on the left hand side
5. Complete all required forms listed

*NOTE: If information is missing, it will not let you submit the form.

Additional Athletic Training Requirements

* Bring a copy (front and back) of current insurance card(s) to the NCAA Compliance Meeting
* ADD/ADHD Medical Exception Form (PDF) – to be completed by prescribing physician and supporting documentation outlined in packet. *Only if applicable
