NEW STUDENT-ATHLETE MEDICAL CLEARANCE

*If you are under the age of 18, a parent/guardian signature is required on several forms. Please complete the online forms as instructed, but contact Head Athletic Trainer, Harry Campbell (harry.campbell@cortland.edu) to receive hard copied of the forms for parent/guardian to sign.

1) Pre-Participation Physical Evaluation Form - This form must be completed by your Primary Care Physician within six months from the start of the season.

2) Sickle Cell Trait Lab Test Results - contact your pediatrician or birth hospital medical records department to obtain a copy of the test results.

    *Please mail or fax PPE Form & Sickle Cell Lab Results to Student Health Services:

3) Immunization & Health History Forms - accessible on myRedDragon - Student Health Services Web Portal (bottom right side of the page under Student Health Services)

    *Please fax immunization records to Student Health Services (607-753-2486). Health history forms are submitted electronically.

4) Athletic Training Forms - accessible on myRedDragon - Student Health Services Web Portal (bottom right side of the page under Student Health Services) - on the left hand side, click on “forms” - complete all required forms listed

    *NOTE: if any information is missing, the system will not let you submit the form until complete

5) Insurance Card - bring a copy (front and back) of current insurance card(s) to the NCAA compliance meeting.

6) ADD/ADHD Medical Exception Form - to be completed by prescribing physician and include supporting documentation outlined in letter.
